School Readiness Assessment Package
A focused consultation package designed to help families understand whether a student is ready for a therapeutic school, transition program, college support program, or postsecondary pathway.
What’s Included
1. Records Review
Submit existing evaluations, school records, IEPs/504s, and/or neuropsychological evaluations for comprehensive review.
2. Initial Consultation Meeting
- A 60–90 minute meeting with parents (and student, when appropriate) to:
- Review current functioning and diagnostic profile
- Discuss school readiness, executive functioning, social-emotional profile, and independent living skills
- Clarify family priorities, preferences, and long-term goals
- Identify whether additional evaluations or supports are recommended
3. Personalized Written Recommendations Report
A customized report outlining:
- Clinical and educational findings
- Recommended school, transition, or college-support pathways
- Program characteristics that may be the best fit
- Strategic recommendations for next steps
4. Strategy Session
A follow-up meeting to review recommendations and create an action plan for:
- School/program applications
- Campus and program tours
- Questions to ask prospective schools and programs
- Timeline and decision-making strategy

Application & Placement Package
Comprehensive guidance for families actively navigating school, college support, or transition program applications.
Designed for families who want expert support through the research, application, and decision-making process without committing to a year-long consulting relationship.

2. Application & Decision Support
Up to Five Additional Meetings with parents and/or student to support the placement process.
Support may include:
- Identifying and narrowing appropriate school, transition, or college-support options
- Discussing the strengths and limitations of different programs
- Preparing for campus tours and interviews
- Reviewing admissions requirements and timelines
- Assistance organizing applications and supporting materials
- Guidance regarding disability services, accommodations, and supports
3. School & Program Collaboration
Consultation with current school staff and prospective programs, when appropriate, to support fit and transition planning.
4. Targeted Consultant Access
Email access for questions and decision support during the application process.
5. Final Decision Strategy Meeting
A structured meeting to evaluate offers, compare options, and create a transition plan.

One-Year College & Transition Planning Package
A concierge-level consulting package for families seeking intensive support through the college, therapeutic program, or transition planning process.
This package provides year-long guidance to help students successfully identify, prepare for, and transition into the right postsecondary setting.

- Everything in the Application & Placement Package, plus:

3. Up to Ten Additional Consultation Meetings Throughout the Year. Support may include:
- Ongoing consultation as student needs evolve
- Evaluating and revisiting program options
- Collaboration with current school teams and future programs
- Building support systems (executive functioning coaching, therapy, academic supports, independent living services)
- Guidance navigating disability services and accommodations
- Supporting student motivation, independence, and buy-in
4. Priority Consultant Access
Direct access to your consultant for guidance, troubleshooting, and decision support throughout the year.
5. Student Readiness Planning
Strategic recommendations to strengthen areas of need prior to transition, including referrals to outside therapists, coaches, tutors, and independent living supports when appropriate.
6. Transition Support
Guidance during the actual transition to help families anticipate challenges and increase student success.
